Before Your Visit...
There are several items to address before your visit.

1. The most important is the Patient Health Questionnaire . It is important that you print this out and fill in to bring it with you to your first visit.
2. Please review our Notice of Privacy Practices.
3. Please bring a list of your current medications and dosages.
4. Our office is a fragrance-free facility.  We discourage the use of heavily scented lotions or perfumes and essential oils as this can cause severe reactions for some of our staff.  You may be sent home if your scent is too strong. Thank you.
5. Please arrive 15 minutes prior to your scheduled appointment for registration purposes.
